Heart failure is a chronic condition that affects millions of people worldwide and can lead to hospitalisation and death if not managed properly.
Auditing the impact of heart failure standards of treatment on clinical and cost outcomes is an important aspect of managing the disease. Currently, auditing is often performed manually and can be time-consuming and costly. Healthcare professionals use various measures such as hospital readmissions and mortality rates to evaluate the effectiveness of heart failure treatment. However, manual auditing can be prone to errors and inconsistencies, and it may not capture all relevant data.
Our poster showcases our ability to identify patients with heart failure and leverage aggregated patient data to extract actionable insights that will improve clinical and cost outcomes.
